How to Add Problem Details to a Formal RCFA Analysis
Adding the Problem Details
The Problem Details are the functional description of the Undesirable Event/Failure. It includes:
- The definition of the function that was affected by the undesirable event - What is the
equipment, process or system supposed to do or what is the equipment not doing
because of the Undesirable Event?
- The impact on production (including loss of production) or operations. The impact on production including loss of production
(totally/partially), product or service quality, customer service, lost sales, downtime, etc.
- The impact on the environment. The Environment can be impacted in different ways such as:
- Toxic release off-site with detrimental or permanent effects.
- Toxic release off-site with no detrimental effect.
- On-site release contained without outside assistance.
- On-site release immediately contained.
- The impact on Safety. Safety can be affected in various ways such as:
- Fatality / Permanent disability.
- Extensive injuries or illness.
- Medical treatment required.
- First-aid treatment required.
- From the previous step of defining the undesirable event, the user must now navigate to the Problem Details tab.
- As you might have already noticed, the Analysis Summary is always carried over from the previous steps.
- The user can decide to link the failure/incident to the Risk Matrix data to quantify the impact of the failure/incident by selecting the "Show Risk Matrix" radio button to display additional fields. If the "Hide Risk Matrix Data" radio button is selected, the Risk information will not be shown.
Risk Evaluation
- Expand the Risk Evaluation section.
The Risk Matrix
- If the "Show Risk Matrix Data" box is checked, the bottom part of the screen will expand with the linked information from the Risk Matrix.
- Capture the Description of the Affected Function.
- Select the Probability from the dropdown menu. The Probability is the likelihood a failure event will occur. The Description will automatically be populated from the selection on the dropdown as this is data from the basefiles.
Defining the Operational Impact
- Expand the Operational Impact section and use the radio buttons to indicate a Partial or Complete Loss. Partial loss or complete loss refer to the extent of operational impact caused by undesirable events.
- Indicate the Severity on operations as a result of the undesirable event by selecting from the dropdown menu. Understanding the severity assists organizations to prioritize issues and helps to Influences the depth of the investigation.
- Capture any supporting information details.
- Quantify the Loss. This is the financial loss associated with the undesirable event. This could be i.e. loss of revenue, cost of repair or replacements etc.
Defining the Physical Impact
- Expand the Physical Impact section.
- Indicate the Severity of the physical impact by selecting from the dropdown menu.
- Provide further details regarding the physical impact by capturing any supporting information to assist with the investigation as illustrated below.
- Now navigate to the Notes and Progress Details section and capture any additional notes regarding the Problem Details. the progress bar will automatically be completed once all the mandatory fields have been captured.
- Click the Save button at the top of the screen.
