How to Set-up the RCFA Failure Patterns or Trends Base File

Failure Patterns and Trends Base File

By understanding how items fail, effective solutions can be developed. This Base File is used to register standard codes for the pattern or trend of failures, events, or incidents.

Adding Failure Patterns

  1. Select the Base files link on the ACTOR index page.


  1. Navigate to the Patterns or Trends tab, click the Add button as illustrated below and a pop-up window will be displayed.


  1. Capture the Pattern and the Description.
  2. Click the Save button.

Editing Failure Patterns

  1. Select the Pattern you wish to update.
  2. Click the Edit button.

  1. Update the Pattern and the Description.
  2. Click the Save button and the changes will be reflected on the grid. 
  1. Select the pattern you wish to Delete.
  2. Click the Delete button and the pattern will be removed from the grid. (Add delete confirmation and toast message for deleted successfully)
