How to Complete the RCM3™ Decision Worksheet

After the Functions, Failed States, Failure Causes, Mechanisms, Effects and Inherent Risks are completed, the RCM3™ Decision Diagram is used to guide toward a failure management strategy. The user answers all questions based upon the Failure Cause, Mechanisms and the Failure Effects completed on the Information Worksheet in previous steps.
The Decision Worksheet leads the user through the different steps to determine the most effective risk management solutions for the Failure Modes identified during the analysis.

NB* It is imperative for the user to ensure that the Information Worksheet is "SAVED" and that the Failure Mode is selected in the tree to activate the next tabs in the RCM3™ workflow. The user will NOT be able to proceed with the analysis if these two steps are not adhered to. The tabs will be greyed out as illustrated below.



Policy Types

Depending on the answers to the questions in the Decision Diagram, one or more of the below listed risk management strategies will be recommended.





REG or Regulation Task
 If it is a regulatory task, then check the “Regulation Task” checkbox and the decision diagram will not apply.



The Decision Diagram

  1. Start with the first (black) box and the boxes will fill out in a order that follows the decision diagram that is generally top to bottom. Some columns may end their line of questioning before all questions have been asked to better mimic the flow of the Decision Diagram. Below is an example of a completed "Decision Diagram".


  1. Once the top row is determined, you are directed to the next question. Each question has a technically feasible and worth doing section. If you want more detail then select the "More" button and further questions will appear, use the directional arrows as highlighted below pictures to reveal more questions.
  2. Click "Done" to close the pop-up box.






  1.  Capture any Notes in the Notes Details section.
  2. Click “Save” and go to the Policy Worksheet tab. Depending on the “Policy Type”, the form will prompt the user to capture different information.
